High Alert: If your child has any asthma and/or allergies to foods or any other health condition you must notify the school, so that a 504 plan can be completed. In addition, you must have the two forms completed and signed by your doctor. Medication that is not accompanied by a doctor's order cannot be given to students. Please call the school (773/534-4100)for further information and ask to speak to the nurse or the case manager.

Parents that want to bring treats for celebrations must adhere to the following:

Brentano is a peanut and nut free school.
No home baked products will be accepted.

When bringing treats contact your child's teacher ahead of time to set up a date and time.

Check with teacher to find out if there are any other allergies in the classroom(eggs, milk, etc.)

Medical Compliance

All students entering CPS for the first time must have a current physical and full immunization record at the time of registration.

All students entering kdg. and all 6th graders must have a current 2011-2012 physical examination.

Dental Examinations are required for all kg, 2nd and 6th graders

Vision Examination is required for all kg.

 

The vision examination requirements are as follows:

Entering the State of Illinois for the first time at any grade level

Entering Kindergarten
